Job Description

Lead care coordination for a caseload of individuals, developing and overseeing nursing care plans, supervising delegated nursing tasks, and serving as the clinical point of contact for families and staff.

Requirements

Active RN license in the state of Maryland. 2+ years of nursing experience, home health or IDD services experience preferred.
